Proforma 4,5,7,8,9----1.Entry fields മലയാളത്തില്‍ ആക്കിയിട്ടില്ല, 
2.Information message for submit button----സേവ് വിജയകരമായി പൂര്‍ത്തീകരിച്ചു/Saved Successfully എന്നത് മാറ്റി "സമര്‍പ്പിക്കല്‍ വിജയകരമായി പൂര്‍ത്തീകരിച്ചു"/"Submitted successfully" എന്നാക്കണം.
3. Proforma submit ചെയ്തു കഴിഞ്ഞാലും Submit button active ആണ്.
