Death_Report_Certificate

1.	Verification Report --Informant’s details(FO)---English ല്‍ രേഖപ്പെടുത്തിയിട്ടില്ല.(Checked with old records. Not included) (ഇതുകൂടി ഉള്‍പ്പെട്ട് വരുമ്പോള്‍ II page blank issue വരുന്നുണ്ടോ എന്നത് പരിശോധിക്കണം)—PDE-Ok
2.	Verification Report—FO---Statistical Info (Continue)---Yes/No ല്‍ No mark ചെയ്തിരുന്നാലും report ല്‍ രേഖപ്പെടുത്തിയിട്ടില്ല എന്നാണ് വരുന്നത്.
3.	Address of deceased—Permanent/Present, Death Place, Informat Address(PDE) എന്നിവിടങ്ങളില്‍ ward select ചെയ്യാതെ House number നല്‍കിയാല്‍ verification report/certificate ല്‍ house number ന്‍റെ ഭാഗത്ത് ആദ്യം / വരുന്നു
