Ward Number: Label to be changed to ward number and provide * mentioning mandatory field. when characters are given page gets hanged up. Must mention which ward number is needed. Ward number has no character limit. Good to have a combo filling current LB wards. 

Death cause details are mandatory which must be identified (* to be given), font must also be changed as others or bolder.
