1. 'Male Dependent' എന്നത് Husband select ചെയ്തു save ചെയ്താലും Annexure ൽ Father എന്നാണ് കാണിക്കുന്നത് 


2. Operator ലോഗിനിൽ ഇത് വീണ്ടും Husband ആയി കാണിക്കുന്നുണ്ട്. എന്നാൽ അപ്പ്രൂവ് ചെയ്യുമ്പോൾ Daughter of എന്നാണ് വരുന്നത് 


3. Ack യിൽ 'Name and Address of the Applicant:' എന്ന ഫീൽഡിൽ Applicant Name & Comma മാത്രമേ (മലയാളം) കാണിക്കുന്നുള്ളു. അഡ്രസ് കാണിക്കുന്നില്ല. അഡ്രസ് Mandatory part ആണ്

5. Annexure – 23 യിൽ മരണസ്ഥലം എന്നത് Main place  മാത്രമാണ് കാണിക്കുന്നത് 



6. Death Place - Home നൽകുമ്പോൾ അവിടെ നൽകുന്ന അഡ്രസ് Ack യിൽ 'Name and Address of the Applicant:' എന്ന ഫീൽഡിൽ കാണിക്കുന്നു. എന്നാൽ Death Place - Hospital ആണ്  നൽകുന്നതെങ്കിൽ ഇവിടെ  Applicant Name & Comma മാത്രമേ  കാണിക്കുന്നുള്ളു.
